FAQs

Do I need to see my Doctor first or bring a prescription for physical therapy?

The state of Oregon allows for anyone to receive a physical therapy evaluation and treatment from a licensed physical therapist without a referral.  We are happy to send your primary care physician written progress with a signed consent.

What can I expect on my first visit?

The first visit is an initial evaluation.  It will involve taking a medical history and discussing current problems, pain, daily activities, functional limitations, medications, tests, and physical therapy goals.

If you haven’t already downloaded and completed the forms, please plan to arrive to your appointment 15 minutes early to complete the paperwork need to start your treatment.

Why doesn’t Functional Movement & Rehabilitation accept insurance?

Most insurance companies will stop reimbursement as soon as they deem you functionally independent, even though you may not have reached your personal activity goal.  In addition, many insurance companies limit the amount of physical therapy that they will reimburse, regardless of diagnosis and age.  I have found this to be one of the biggest obstacles in patients reaching their highest functional level.

Having a cash-based service allows me to provide you with individualized and preventive care so that you can reach the goals that are important to you for your quality of life, independent of the restrictive regulations set by the insurance industry.

I accept Personal Injury Protection (PIP) claims.  Otherwise, I accept cash, check or credit cards.  I will be happy to provide an invoice so that you can submit for reimbursement from your insurance company as applicable.
